1. Organizational tool only
The service helps fitness professionals organize information, screening responses, coaching indicators, program options, and follow-ups. It is not a health care provider, medical device, clinical decision-support system, diagnosis, treatment plan, medical record, or substitute for advice from a qualified health professional.
2. Coaching indicators
Percentages, readiness summaries, opportunity areas, recommendations, and similar outputs are coaching aids based on entered information. They are not validated clinical scores, risk predictions, guarantees, or instructions to begin or continue exercise. Missing answers are shown as incomplete and should not be interpreted as a favorable result.
3. Professional responsibility
Fitness professionals remain solely responsible for their services, scope of practice, screening, referrals, program design, communications, and decisions. They must use independent professional judgment and comply with licensing, consumer-protection, privacy, recordkeeping, and informed-consent requirements that apply to them.
4. Participant responsibility
Participants should provide accurate information, stop activity that causes concerning symptoms, and seek qualified medical advice before beginning or changing exercise when appropriate. Information in the service does not create a clinician-patient relationship with MDG.
5. Sensitive information
Only enter health or wellness information that is relevant, authorized, and lawful to collect. The service is not represented as HIPAA compliant. If a business is a HIPAA covered entity or business associate, it must not use the service for protected health information without a separate written agreement and confirmation from MDG that the required safeguards are available.
6. No outcome guarantee
Fitness, business, sales, health, and behavior outcomes depend on many factors outside the service. MDG does not guarantee any particular result from use of the application, its templates, or its coaching indicators.
